Welcome to Active Care Wellness Center!
Whether you’ve had chiropractic care and/or physical therapy before, or it is your first time, going to a medical office can be overwhelming, and sometimes even intimidating. This page was created with that in mind. We want to make your experience with us as comfortable and as streamlined as possible.
PREPARING FOR YOUR VISIT
BEFORE YOU ARRIVE
Before you arrive for your appointment, there are a few things you’ll want to have with you:
- Driver’s license
- Health insurance card*
- List of current medications/vitamins and dosages (if applicable)
- Any diagnostic imaging you’ve had done (if applicable)
- New Patient Paperwork (if you’ve chosen to print and fill them out at home)
*We do offer a cash discount if you do not have health insurance, or if your health insurance doesn't cover chiropractic or physical therapy. For more information, please call us at (406) 777-1048.
Upon entering our office, our wonderful office staff, Laura and Nancy, will greet you with a warm welcome (and don’t be surprised if Eiger, our loveable Bernadoodle office mascot, greets you as well and requests pets). Our staff will request a copy of the documents listed above. At this time, if you have health insurance, Laura or Nancy will look up your chiropractic/physical therapy benefits and let you know what they are, and give you a ballpark estimate of your initial visit’s cost (if applicable).
If you haven't already filled out the New Patient Paperwork at home, our staff will provide you with a copy to fill out in the lobby prior to your being seen by the doctor(s). This paperwork provides us with your health history and information on your condition. If you prefer to fill out the forms in the office, please arrive 15 minutes early to complete the paperwork.
DR. AMY BERGLUND’S PATIENTS
Once your paperwork is complete, one of our staff will take you on a mini tour of the office on the way to the exam room. Our building (Headwaters Wellness Center) is home to a few different providers who occasionally collaborate with Dr. Amy to provide optimal care for our patients.
Once you are settled into the exam room, either Laura or Nancy will take your vitals, ask you a few health history questions, and go over some quick insurance paperwork (if applicable) before Dr. Amy arrives.
Once you are in your private exam room and the new patient paperwork is completed, Dr. Amy will begin your exam. She will start by taking an in-depth history of your current condition followed by past medical history and lifestyle. Next, she will start the physical examination, which will consist of neurological and orthopedic tests. This will allow Dr. Amy to know all aspects of your condition. If Dr. Amy has any concerns during the examination, she may choose to order radiographs or an MRI. We do not take x-rays in-office.
REPORT OF FINDINGS
Once the information is collected and examinations are performed, Dr. Amy will give you a detailed report of all findings and answer any questions including:
- How can Dr. Amy help me?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health history, exam findings, and personal goals, Dr. Amy will discuss recommendations and notify you if your condition requires care with other providers. Dr. Amy will provide the best treatment and wellness program for your needs.
Once it has been decided that Chiropractic is the preferred method of treatment, you will have your first adjustment. The first treatment can include spinal manipulation, physical therapy, and/or soft tissue massage. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.
Prior to leaving, Dr. Amy will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office, this may include ice or heat application instructions, certain activities or positions to avoid, and at-home exercises and/or stretches. If you desire, our wellness team will work with you to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
